These summaries are very useful for the Care Team and Doctors to analyse the current medical state a Patient is in. They can create new alerts for the patient, assign tasks to other team members, etc.
The Care Team and Doctors have 24hr access to a monthly report which clearly shows improvements and/or areas of concern they need to address with their Patient.
Patients can see their overall progress and track their success in reaching the goals they set for themselves. The better they get at taking daily readings, walks, etc, the higher their status will become.
Patients can add and manage the medicine they are on and Doctors can always see it and advise accordingly.
Patients are able to setup a video call with their Care team and vice versa, at any time.

We all need a bit of daily motivation, but when it comes to Chronic Disease Management, these messages can save lives. James A. McClung is a recognized expert in accelerating profitable growth and managing results-oriented businesses, the corporate governance required to operate them, and the development of global executives. He has established a consistent reputation for bottom-line and turn-around management excellence across a diversified portfolio of chemical, machinery, consumer, and service business sectors in more than 75 countries, both developed and developing.
His corporate board of directors’ experience includes seven private, public and family-owned companies. In addition, he was a founding member of the U.S.-Russia Business Council, and currently is active in other global business organizations, including the Japan America Society, Economic Club of Chicago, Executives’ Club of Chicago, and the Chicago Council on Global Affairs. He also serves as a trustee for a several higher education boards.
McClung earned his bachelor’s degree from the College of Wooster, and received their Distinguished Alumni Award in 2014. He also holds a master’s degree from the University of Kansas and a doctorate from Michigan State University.
James was Chief of Thoracic Trauma at the Charity Hospital in New Orleans. He became a partner in a new Practice in South Bend, Indiana in 1986. They started a new Practice at Saint Joseph's Hospital in 1989. Five years later they started a Cardiothoracic program at Elkhart General, thus covering the #3 major hospitals in North Central Indiana. By 2005, they had achieved his own personal goal becoming a Top 100 Cardiac Surgical Program at Memorial Hospital of South Bend in its initial #3 years of ratings for Valve Replacement and Coronary Bypass Surgery.
He completed an MBA a the Kellogg School of Management in 2007. He has joined three Angel groups - Magnet Investors, Heartland Angels, and TIE Midwest.
Prior to joining Sg2, Jay was a faculty member at the Northwestern University Feinberg School of Medicine (NUFSM), where he served as a professor of surgery. He was also an attending surgeon at Northwestern Memorial Hospital, where he provided clinical expertise and leadership in minimally invasive and bariatric surgery. Jay served as chief of surgical services at the Jesse Brown VA Medical Center in Chicago, IL, from 2005 to 2014. In this role, he provided administrative, clinical and financial oversight to ~70 clinicians and staff who performed ~3,200 operations and ~21,000 outpatient visits per year.
Jay received his MD from the Pennsylvania State University College of Medicine in 1983. He completed his general surgery residency training at NUFSM in 1989. He also holds a master of business administration from Northwestern University Kellogg School of Management and a master of health professions education from the University of Illinois at Chicago. He is certified by the American Board of Surgery and is a fellow of the American College of Surgeons.
Dr. Amar Hamad is a Medical Oncology Specialist and the CEO of Affiliated Oncologists. Dr. Hamad has more than 32 years of diverse experiences, especially in Medical Oncology, Hematology/Oncology and Internal Medicine. In addition to cooperating with other doctors and specialists in the medical group Affiliated Oncologists, LLC, Dr. Hamad also affiliates with many hospitals including Advocate Christ Hospital & Medical Center, Silver Cross Hospital and Medical Centers, Palos Community Hospital, Little Company of Mary Hospital and Ingalls Memorial Hospital.
Joanne Glenn, born and raised on the Southside of Chicago is the President & C.E.O. of Women on Top of Their Game, Inc. Foundation, (W.O.T. Foundation).
W.O.T. Foundation provides a platform for health education, mentorship and facilitates opportunities through a myriad of networks. WOT Foundation advocates for Breast Cancer Survivors addressing the gaps in health care and best practices. WOT Foundation Provides an environment for information sharing, Chef prepared meals and advocacy that supports and discuss best practices and options through the journey of Survivorship. Glenn has been recognized as an expert in the field because of dedication, experience and hard work. Bringing a wealth of leadership in Health care, Administration, Community, Advocacy & Policy and Philanthropy. Glenn a product of Chicago Public School system, a graduate from Wendell Phillips High School and inductee into “The Hall of Fame”, received her Nursing Diploma from Michael Reese School of Nursing , Bachelors in Science, (BS) from National University , Master in Business Administration, (MBA) from University of Phoenix , and Published in Journal of American Medical Association (JAMA) for research in Home Blood Pressure Study. Co-producer of a mini docu-series “Everyday is October” 2013, (www.everydayisOctober), is working for a healthier tomorrow.
Cathy has a BSc in Healthcare administration with more than 25 yrs. of experience with management and program development in many areas of cancer care including surgical, radiation and medical oncology, palliative and hospice care.
Accomplished and decisive health-care executive with history of increasing sales and profits, turning around under-performing divisions and leading expansions for multi-unit operations, with focus on quality value-based patient care. Combine expert clinical experience, strong team-building and refined interpersonal skills with decisive leadership qualities to guide organizations through change, growth, profitability and performance improvement. Strong communicator; relates easily to all levels of patients, staff, executive administration and corporate teams.
Her education includes a BSc, Healthcare Operations/Administration (1996) at the University of St. Francis Joliet, Illinois, RN Licensure (1986) and a Diploma (1986) from the South Chicago School of Nursing. Cathy is currently a member of the Association of Community Cancer Centers, the Illinois Medical Oncology Society, the Advisory Board Oncology Roundtable and the American Association Oncology Administrators.
